You are welcome to Limi Hospital and Maternity Ltd. We are a Patient Centered Family Hospital, and will be happy to be of service to you.

 

Registering at The Limi Hospital

 

New patients are always welcome. We are happy to have you join The Limi Hospital family. Registration at the Limi Hospital can be done at our Central Outpatient Reception Desk.

 

This reception is located on the ground floor of the A-Wing of the Hospital (which is the building to the right of the Hospital gate.

 

Our Client Service Managers are cordial, friendly and will promptly assist you through your hospital visit.

 

You will be required to fill out a patient registration form. This form collects personal information and other required information that will be necessary for your management and care while at The Limi Hospital. Please fill out this information carefully and clearly.

 

 

Under Health Insurance/Retainer Cover?

 

Our Retainer Clients and Health Insurance patients are welcome. We appreciate you all for choosing The Limi Hospital to take care of your staff and their families.

 

If you are a member of any Health Insurance scheme or a Retainer cover, you are required to inform the Client Service Manager at your registration and during each hospital visit.

 

Please be informed that the Hospital will not liable for refunds where this information is not supplied.

 

At the point of registration, the following will be required:

- Health Insurance Identity Card (this is required for all hospital visits)

- Filled Patient Registration Form

- Referral form (where required)

- Letter of Introduction (for retainer clients, where required)

- Stamped and Signed Hospital Attendance form (for retainer clients, where required)
